Age Bold

Medical Director - Multi-State License

Los Angeles, California, United States

About the role

Age Bold, a digital health platform delivering personalized exercise and preventive care programs to Medicare beneficiaries, is hiring a Medical Director to lead clinical operations across multiple states. The role blends hands-on patient care with growing leadership responsibilities, starting with 60-70% direct telehealth delivery focused on older adults and lifestyle medicine, then gradually shifting toward oversight as the company scales. You'll also spend 20-25% managing and mentoring clinical staff, and 10-15% on quality improvement, compliance, and governance—including serving as the designated Medical Director for CMS innovation model participation. This position suits experienced physicians (MD/DO) in internal medicine, family medicine, geriatrics, or lifestyle medicine who want to remain clinically active while building systems and influencing product direction. You'll need active, unrestricted licenses in California, Texas, and Pennsylvania (with multi-state licensure preferred), 5+ years of clinical experience with at least 2 years in a leadership capacity, and strong telehealth and EMR skills. Background in preventive care, weight management, chronic disease, and musculoskeletal conditions is valued, along with comfort operating independently in a startup environment. This is a full-time remote position based in Los Angeles with compensation of $220-240k annually based on experience. Age Bold covers state license expansion, malpractice insurance, comprehensive health benefits, and 12 weeks of paid parental leave. You'll join a venture-backed, mission-driven team working on one of the first CMS-approved digital MSK programs in the country.

  • Conduct regular provider meetings to discuss clinical protocols, complex cases, and updates.
  • Act as a liaison between the clinical staff and the administrative/business operations teams.
  • Be the voice of the practicing provider in product design.
  • Oversee the recruitment, onboarding, and professional development of new clinical staff.

Quality Improvement, Governance & Operations (10-15%)

  • Collaborate with executive leadership to develop and implement clinical policies and procedures.
  • Lead Quality Assurance and Performance Improvement initiatives to enhance patient safety and care delivery.
  • Ensure the facility maintains compliance with all local, state, and federal healthcare regulations.
  • Serve as designated Medical Director under CMS innovation model participation, responsible for clinical policy authorship, outcome monitoring, and pre-submission data validation.

  • MD/DO in Internal Medicine, Family Medicine, Geriatrics, or Lifestyle Medicine.
  • Active and unrestricted medical licenses in CA, TX, PA. Multistate license preferred.
  • Active individual Medicare enrollment in good standing.

  • 2+ years experience in telehealth with high proficiency with EMRs & communication tools.
  • Strong foundation in preventive and lifestyle-focused care including management of weight, hypertension, diabetes. Familiarity with MSK conditions and outcome measurements preferred.
  • Ability to balance clinical and administrative priorities, and a commitment to data-driven quality improvement.
  • Excellent communication skills and empathy-driven clinical approach.
  • Comfortable working independently in a fast-paced startup environment—you thrive in ambiguity and enjoy building systems and processes from the ground up.

Why You’ll Love Working With Us:

  • 100% remote work.
  • Be part of a mission-driven healthcare business passionate about transforming care for older adults.
  • Work alongside a tight knit clinical team to help build the clinic and shape the future of lifestyle oriented virtual care.
